RespAir Sleep (“we,” “us”) respects your privacy. This policy explains how we handle information collected through our website (the “Site”).
Website and secure forms
Information you choose to share
Our website provides information and links to, or embeds, RespAir Sleep’s Jotform forms. These include consultation requests, clinician referrals, insurance questions, and sleep questionnaires. Depending on the form, you may provide contact information, health-related answers, or records relevant to your care.
The clinician referral form is intended for authorized offices to securely attach sleep studies and clinical notes. The contact form is for initial inquiries; our team can direct you to the appropriate secure workflow for detailed records or intake documents. Please do not send identification or insurance-card images by ordinary email.
Where form information goes
Form entries and attachments are submitted through our HIPAA-configured Jotform workflow. This website does not itself provide a patient-record database or a separate patient-information intake form. We use information you submit to respond, coordinate referrals, schedule care, and support clinical and insurance workflows.
Limited website analytics
We use Google Analytics 4 only on our homepage and Our practice page to understand general website visits and basic engagement. We do not load Google Analytics on our contact, referral, questionnaire, insurance, first-visit, treatment, article, appointment-confirmation or authenticated pages, or on sleep.respairsleep.com.
Google receives technical information when these two pages use Analytics, including your IP address, browser and device information, cookie-based identifiers, and the page viewed. This is not anonymous measurement. Our configuration sends only the approved page address and a fixed page title, without URL query strings, fragments or referring-page addresses. Analytics cookies are configured to expire after 30 days without extending that expiration on each visit.
We disable Google signals, ads personalization, user-provided data collection and automatic measurement of searches, forms, outbound links, downloads, videos and scrolling. We do not send names, email addresses, phone numbers, form answers, medical information, call events, consultation outcomes or treatment outcomes to Google Analytics or advertising platforms through the website. We do not use Meta Pixel, Meta Conversions API or Google advertising conversion tags on these websites.
Source attribution and practice records
We use source-specific phone numbers and source or campaign labels associated with Jotform inquiries to understand how people find us. These labels are retained in this browser tab for up to 24 hours and may accompany an inquiry submitted to our practice. Ad click identifiers are not stored in those inquiry fields. Phone numbers displayed on the website may vary for Google Ads, Meta Ads, Google organic search and other visits.
Our team reviews inquiry and call records in our practice systems. We assess marketing using aggregate reports of confirmed inquiries and treatment starts; the website does not upload those records or outcomes to Google Analytics, Google Ads or Meta. A form submission or phone-button click alone does not establish a qualified inquiry.
Your analytics choice
You can turn off limited Google Analytics in this browser below. We also honor a previously declined measurement choice, Global Privacy Control and Do Not Track. Your choice does not prevent you from using our forms or calling the practice. Clearing browser storage can clear your saved choice.

Service providers and technical information
The hosting and form services needed to operate the website may process technical information such as IP addresses, browser details, and security logs. Jotform handles submitted form information under the practice’s configured service arrangements. We do not sell your information. Information may be shared with service providers as needed to operate these workflows, or as required by law.
